What Does Anterior Glenoid Bony Loss With Focal Chronic Antero-inferior Labral Tear, Showing Stripping Of Anterior Scapular Periotism Mean?

in last one year my shoulder has been displaced 5 times , and I can not rotate my hand back word direction. . MRI result is below : 1. large hill sach s defect of 1.9 cm in length and 3.5 mm in depth at the posterior superior aspect of Rt humeral head. 2. Anterior glenoid bony loss (20mm in length and 3.5 mm in depth) with focal chronic antero-inferior labral tear, showing stripping of anterior scapular periotism - Perthes lesion. what is the best solution for my problem?
