a 36-year-old male presents to the emergency department complaining of fever and chills on and off for the past week. He also complains of bloating and diffuse abdominal pain 4/10, but goes up to a 9/10 when someone touches it, a dry cough, and a headache that only hurts in the front of his head. Today his fever was 103. His wife states he hasn’t been eating anything lately nor has he been acting like himself. He has a family history of hypertension, stroke, and diabetes. He is having confusion and hallucinations at times.
