When these chronic micro organisms enter in the lungs, they are entrapped by inflammatory cells surrounded. And this inturn form rounded shell like structure known as granuloma. You are having hemithorax in the past. So possibility of tuberculosis is more in your case. So better to consult pulmonologist and get done CT thorax with Contrast, Bronchoscopy with BAL (bronchoalveolar lavage) and biopsy to rule out tuberculosis or other chronic infection.
