Hello, Thanks for consulting HCM.
ESR is an indication of some chronic infections, some parasite infection and some Systemic diseases where it rises to significant amount. But the level of 118 is too high and there is possibility of having tuberculosis. The other indicator is X-ray which shows collapsed lung, fever and cough for that long. I would suggest to get hold of a chest physician and get evaluated, he may suggest of a CT scan of chest and sputum test. Such a long standing fever and cough has to be evaluated soon. Do not consider his age as deterrent - and get it evaluated and if found positive - it can be treated completely, and also he would be symptom free in a week of starting the treatment.
Wishing for his quick recovery.

HI, thanks for using healthcare magic

Contraction of the lungs means that they have decreased in size. It may occur due to different causes including infection of the lungs.

The persisting cough and fever indicate that he may have a persisting infection in his lungs.

He may need a course of antibiotics to help treat the infection.

Physiotherapy may also help to restore the size of the lung.
Your doctor may refer you to someone who can help.

I hope this helps
