What Does Inter Menstrual Bleeding Signify?

ive been getting 2 periods a month. My first cycle starts at the end of the month and the second one begins in the middle. In between each cycle I have a lot of discharge - it s not fishy smelling but it does have a smell to it as well as its very wet. It s been happening for a few months, last month it didn t happen but it seems to be happening this month. I went to a gyno and they were incredibly unhelpful. One told me this was normal and another told me it was not but did not tell me what to do. She also told me I have bacterial vaginosis but did not prescribe me anything only told me to take flaxseed oil. This doesn t sound right to me. I m at my wits end with this and im scared that something more might be going on, should I go to emergency? There are no doctors available where I live, only walk in clinics and that s where I got 2 seperate answers from 2 gynos who clearly had better things to do than to help me.

Hello,

Many factors can cause short cycles. I would like to know a few more details to confirm. Your age, any comorbidities like thyroid, contact history. Also, regarding your vaginal discharge, I would like to know whether it is accompanied by pruritis, color and also consistency of the discharge. Your short cycles needs evaluation.

If you have been getting these short cycles for a couple of months you should do a complete blood count (to check for anemia due to excessive blood loss),TSH and prolactin level in blood (both can cause irregular cycles if values are abnormal). It is worth doing an ultrasound scan to look for any uterine or ovarian cause for short cycles. Even a biochemical pregnancy/miscarriage can cause irregular bleeding.

Regarding vaginal discharge if you have not done a pap smear in last 1 or 2 years you can do it to rule out any pathology of the cervix. You can also check your blood sugar levels which if high can cause recurrent candida infection of vagina. If your discharge is pruritic, thick and curdy it could be due to fungus. Clotrimazole vaginal tablets can help. If your discharge is greyish or greenish and itchy it could be bacterial vaginosis which can be treated with clindamycin vaginal tablet or oral metronidazole.

These are the 2 most common causes of vaginal discharge. If you can specify the details of discharge i can answer more specifically. If possible you can get the blood investigations done (CBC, TSH, Prolactin), ultrasound pelvis for evaluating short cycles. You can try using Clotrimazole, Clindamycin vaginal tablet if you have the type of discharge that i have specified.
