What Does Lump Appearing On Anus During Bowel Movement Suggest?

My husband when he open is bowels there is a lump that he pushes back in. He is also suffering from back pain . He has done a dip test and this is showing a uti , however he his more concerned with the lump and the leakage he his getting from his back passage. Thanks Lou
