Welcome to health care magic.

1.The report suggest focal scarring - which represent previously healed infection shows a trace as scar.
2.Pyleonephritis and glaomerulo-nephritis can be the cause - urine pus cells also suggest presence of bacterial infection.
3.Left ovary size appears normal with a dominant follicle of normal size for fertilisation noted.
4.Treatment in this case will be a course of antibiotic course will be helpful.
5.In severe cases an biopsy will be helpful to assess the actual cause. Good luck.

Hope i have answered your query,
