Hi,

Incomplete data like age, other details of semen analysis, total volume 18 ml? As per WHO guidelines the normal volume of the seminal fluid should be 1.5 ml or more, the sperm count should be 15 millions/ml or more and progressive (active) sperms motility should be 30%.

Based on the report given your sperm count is less than 10 million/ml and the active motility is less than 20% which seems to be due to grade 2 varicocele. I advise you to consult a surgeon for possible surgical treatment for varicocele following which your sperm count and the motility will also increase thus increasing the likelihood of pregnancy.

The earlier you start treatment, the better your chances of improving sperm production. You also need to have certain lifestyle modifications such as:

1. Avoid smoking, alcohol, coffee and stress.
2. Do regular exercise and try to lose your body weight if you are obese.
3. Wear loose cotton undergarments, avoid hot baths, laptops, saunas.
4. Take nutritious diet such as sea foods, fresh fruits, meat, eggs and vegetables.

Hope these answer your query. Let me know if I can assist you further.

Regards,
Dr. Mohammed Taher Ali
