Hello. I am a 42yr old white female, 5 4 , 225lb and recently had an ultrasound that showed an enlarged liver (18.4 cm) with some fatty sections and enlarged spleen (12 cm). I went to my gastroenterologist and he said that the liver was not that large as 16 cm is normal. I have read that 7cm is the normal - can someone give me an opinion on that?

This means that your size of up to 16 is not significantly above this.
It is possible that the build up of fat in the liver can cause problems if it continues since non alcoholic fatty liver is fast becoming one of the common causes of significant liver disease.
Lifestyle changes in diet, exercise , as well as the use of medications such as statins would help prevent progression.
