Welcome to Health care magic.

1.The poorly defined opacity - possible cause could be come collection ( inflammatory / infectious ) with in the alveolar spaces. other possible cause could be localised effusion, lastly mass lesion.
2.Bronchial wall thickening is seen in case of inflammation.
3.A course of antibiotics will help to resolve the infection.
4.If it does not helps - a high resolution computer tomography will help to evaluate exact cause of the opacity - collection / localised effusion / mass.
5.Its good to stop smoking, it helps in early recovery, and in long standing helps in healthy life style. Good luck.

Hope it helps you. Wish you a good health.
