What Does Pain Around A Hip Surgery Incision Site Indicate?

Hello - I had full hip replacement surgery on 3/11/16, over 18 months ago. My incision area is warm to the touch. I have pain in that area if you press it as you would a doorbell. I screamed when my doctor pushed on that area about 4 months ago. It was so painful that I literally cried right then. My entire outside left thigh (the leg the hip was replaced) is completely numb. I frequently have electrical stings/needle pricks on top of my left thigh. And, the upper portion of my left hip is very painful, literally 24/7. It hurts immensely when I bend over to pick something off the floor/ground and then raise up. I have little strength in my left leg, able to raise it 3-4 inches while laying down. I have had a CT scan, MRI, blood tests, xrays, 35 physical therapy appointments, etc. I saw a nerve doctor 4 weeks ago and he gave me a cortizone shot in the hip area near my groin 2 weeks ago. It has not relieved any pain or helped in any way. My doctor told me that he has performed over 2,000 hip replacement surgeries and has never encountered anything like this. He has told me at least 5 times that he has no idea what is wrong with me.

General Surgeon 's  Response
Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Such a severe pain on pressure on hip replacement area, numbness of part of thigh and pins and stings in some area, increase in pain on bending down all indicate that you have nerve involvement. This can be at the operation site when operated as there is a large incision and thus the nerve can be trapped into fibrosis or pinched. Also needed is to see for the origin of pain, whether local or from the compression in lower spine hence MRI of the spine is also needed. Get a review or a second opinion of another surgeon or Neurologist. Go for proper physiotherapy. Sometimes rhizotomy may be required once you accept the fact that life can be better but there will be numbness forever.

Hope I have answered your query. Let me know if I can assist you further.
Regards,
Dr. T Chandrakant
