General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

The picture is of atherosclerotic disease in heart. The pain is due to myocardial ischemia. To rule out how many and how much of the coronary arteries are affected, he has to undergo angiography.

My advice is to do the test when facility of angioplasty is also available, so if required it can be performed in single sitting.

Hope I have answered your question. Let me know if I can assist you further.

Regards

Dr. Hiren
