For several weeks I have been having pain in the joints at both ends of my left clavicle, a swollen medial clavicle, and pain underneath the mid-section of my left clavicle. I cannot sleep on my left side and cannot raise my arm above my shoulder. It does not hurt, however, to hold a 10-pound weight with my left arm.

This is mainly because of the weight of the arm on the bone. A fracture or ligament injury of the collarbone can occur easily because of a fall on the shoulder or a road accident. I would advise you to check with your serum calcium and Vitamin D levels. Deficiency of such nutrients is the main cause of pain in bones.
Poor posture does indeed have a direct correlation to collarbone shoulder pain and is one of the main causes of joint damage.
Get it checked with your orthopedician.
