What Does Pain In The Neck And Shoulders Indicate?

I have muscle pain from my neck going into both shoulders and upper arm muscles. Sore all day but during the night the pain wakes me up and is very severe. Once I’m up it subsides to just sore muscles. I have HNPP and was also diagnosed with PMR. My doctors say neither of these are related to my conditions. I’ve had this on and off for years and no answers.
