What Does Pituitary Lesion, Potential Pars Intermedia Cyst Or Adenoma Shown In MRI Mean?

So I had an MRI and I am not understanding much of what was found. I always have headaches, anxiety, mood changes and serve fatigue. Can you help? Narrative MRI HEAD AND PITUITARY W AND WO CONTRAST 06/23/2015 HISTORY: Galactorrhea, hyperprolactinemia FINDINGS: On a 1.5 Tesla magnet, with intravenous administration of 20 mL MultiHance, the following sequences were obtained: Sagittal T1-weighted, axial diffusion, FLAIR, T2 and T1-weighted pre-gadolinium, axial T1-weighted post gadolinium imaging of the brain; sagittal and coronal T1-weighted pre- and postgadolinium imaging of the sella, including dynamic postcontrast imaging in the coronal plane. A small mucus retention cyst or polyp is seen in the left maxillary sinus. No paranasal sinus or mastoid fluid. Intraorbital structures are unremarkable. Brain volume is normal, with no signal abnormality, infarct or restricted diffusion. The ventricular system is nondistended. No intracranial fluid collection, mass or pathologic enhancement. Vascular structures are unremarkable. Imaging of the sella is suboptimal due to motion artifact. The pituitary gland is normal in size, with midline stalk. A questionable small hypointense lesion is seen at the to effacement of the anterior and posterior hypophysis on a single series (image 8 of series 13). Suprasellar and parasellar structures are intact. Impression IMPRESSION: 1. Questionable small pituitary lesion, potential pars intermedia cyst or adenoma. 2. Suboptimal evaluation of the sella due to motion artifact.

General & Family Physician 's  Response
Hi,

Based on the MRI findings small pituitary lesion is identified. The differential is either an embryological remanent called pars intermedia cyst or an adenoma or However, it seems the technical aspect for evaluating the lesion was not optimal.

I recommend that you repeat the MRI test in light of those findings with a specific focus on the pituitary gland, ask for dynamic sequences to be used as this will give an idea of how active this small lesion. The size of the lesion is not described in the report which plays a role in the differential.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Muhammed Aslam T. K., Pediatrician
