Welcome to health care magic.

1.The report suggest there is a large filling defect seen in the uterine cavity - which can be some intramural fibroid - one of the possibilities.
2.The other possibilities could be polyp of the endometrium.
3.Both the fallopian tubes shows no spillage - suggest tubal blockage.
4.This condition is seen in case of previous infections / post treatment blockage.
5.In this case possibilities of conceiving are difficult with out treatment, there is procedure to dilate the tubes - you need further investigate.

Good luck.
