What Does Severe Pain Below The Left Breast Indicate?

Hello. Having some pretty bad pain in my left upper quadrant, just below the breast, and on last couple ribs if I sit up using my abdominal muscles or lean left for a while and go to sit up, causes good amount of pain. I know from CT two weeks ago with contrast that my liver, spleen and pancreas are enlarged.

It can be due to splenic infarct or cardiac origin pains if especially radiating to left shoulder and arm.
