What Does Severe Pain In The Left Leg Indicate?

I am having difficulty lifting up my left leg and have pain shooting on the inside area when I do . My MRI said I have grade 3 fibrillation involving the trochlear groove along with a focal grade 3 fussure involving the wieghtbearing portion of the lateral femoral condyle. What does this mean and does it involve surgery?
