General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

If the response to NSAIDs is poor and their is excruciating pain then you should go to an ER near you. Imaging of the C spine would be required after examination. Intramuscular Diclofenac can be given for prompt relief along with Tramadol based on severity.

Hope I have answered your query. Let me know if I can assist you further.
