What Does Severe Pain In The Shoulders Indicate?

I am a 55 yr old overweight female. I have been having some issues with an electrical type pain that shoots across the top of my shoulders. I am usually weak and tired following these episodes. I have a strong family history of heart disease and have had my heart thoroughly checked out within the last 3 years, with no issues found at that time. Should I be worried?
