What Does The ECHO Test Report Suggest?

I m a 73 old male, 6 3 @ 260. Two heart attacks, 5 by-passes, two open heart, 4 stints. Last 6 months very hard to breath. Pulmologist says COPD is Ok. Echo shows 30% extraction factor as it did a year ago. I seem to be getting worse, but Dr. want s only to treat with meds which are not helping.
