Shrunken liver with diffuse surface irregularity means chronic parenchymal liver disease that is cirrosis.there wil be fibrosis is going on inrespibse to insults like alcohol ,viral hepatitis etc.heterogenously enhancing mass lesions in all segments of right lobe of liver with increased activity indicate multi focal hepatocellular carcinoma.that means due to repeatedongoing insult & fibrosis associated with regenerating nodules and thus malignancy some times.
