What Does The Following CT Scan Report Suggest?

Hi A 20 yrs old young man diagnosed as intestinal tuberculosis on the finding of enlarged intestinal lymph nodes he was treated with ATT for three months but there was deteriration in his condition.He was tested for LDH and repeat CT abdominal scan.His LDH was high 1400 and new enlarged lymph nodes in abomen.His B12andfolic acid was also tested.Folic acid was low and B12 was normal,what is ur suggestion.patient is still on ATT

Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

If the patient is not responding for ATT means there are two possilities

1. Multidrug resistant TB - the TB bugs may be resistant to regular ATT drugs. So you may need special drugs
2. lymph adenitis is due to other causes, may be we should exclude lymphoma.

Open biopsy can reveal the correct diagnosis.
