Multilevel degenerative changes are detailed above. There is advanced facet arthropathy at L4-5, which along with shallow disc bulging, results in mild bilateral foraminal narrowing. A facet cyst on the left at this level narrows the lateral recess, displacing the descending L5 and S1 nerve roots. Help me understand my options please. I have exhausted injections. Will a nerve ablation solve this problem or just postpone the need for a more invasive option.
You may try physical therapy or decompression therapy. Use antidepressants and pregabalin or gabapentin may useful. Acupuncture and back strengthening exercises are useful.
If not improved need nerve blocks or subdural steroid injection. If not improved surgical laminectomy or discectomy. Please consult your neurosurgeon/orthopedician he will examine and treat you accordingly.
