Your baby is 4 month old and is having frequent loose stools after 3rd vaccination. I think that is not related to vaccination. Teething could be the primary cause. At her age, she use to bring everything inside her mouth due to teething. This will bring digestion issues.
Furthermore, at this age, you might introduce foods/drinks other than milk. This can alter the function of her delicate gut experienced with diarrhea.
Get her stool culture for candida overgrowth as it can also cause loose stool.
