What Does The Following Lymph Node Biopsy Report Suggest?

My husband met with his doctor to review first ever lung/lymph node biopsy results. the conversation was minimal and we are very unclear on the diagnosis. Today we requested a copy of the biopsy report. The report says, metastatic high grade carcinoma with neuroendocrine features, extensively necrotic . That is what was said about the majority of the lymph node samples (mediastinal, paraesophagial, hilar) The lung tumors say, rare atypical cells present in a background of pigmented macrophages and mixed inflammatory cells. There s more but does this give you any idea of what we re dealing with? My husband has lymph nodes the size of small eggs (we took the pet scan disc to another facility and got to look at it but we didn t have the pet report so that doctor couldn t tell us too much. Also, we didn t have the biopsy report at that time) The doctor we met with at the facility where the biopsy was done said my husband had a rare cancer. He said he has both small cell and non small cell. I pinned him down and said WHAT DOES HE HAVE?? He said Neuroendocrine. Any help you can give me to understand would be appreciated.

Pathologist and Microbiologist 's  Response
Hi,

The biopsy report is showing metastatic cancer. The type and site of primary carcinoma need to be found out. Immunohistochemistry of lymph node will determine the type of a tumor.

If there is clinical suspicion of lung cancer, EGFR and ALK mutation need to be carried out on the lymph node biopsy.

Hope I have answered your query. Let me know if I can assist you further.
