What Does This CT Scan Report Indicate?

I had a CT scan for a pain in my left side two impressions came up and I would like to know what kind of a doctor to go to for them. Antecedent granulomatous disease in the subcarinal region and spleen. moderate elevation of the anterior two-thirds of the right hemidiaphram likely due to chronic hemiparesis. this causes some chronic subsegmental atelectasis in the right lung base
