What is the out come / prognosis of a massive stroke with a midline shift from 4mm to 11mm. CT scan shows the pooled blood in his skull to be like jello while he continues to drain blood. This has been 2 week & 1 day. Unresponsive & unconscious without meds.
I would like to tell you that hemorrhagic stroke has a good prognosis if the patient survives the initial tough time. There is slow recovery. The mid line shift will be back to normal but as you know the clot in brain needs some time to resolve or shrink or dissolve and only then once these pressure symptoms of this big clot / bleed are finished, the patient starts gaining consciousness slowly over few days to weeks. But once he has recovered long term outcome is good.
Meanwhile what you really need to do is to protect patient from developing bed sores by changing his postures 2 hourly and helping paramedics in keeping his pelvic area washed and cleaned at its best (frequent pampers change). This will keep skin healthy and more over he needs physiotherapy as passive exercises to all over his body and a proper regular tube feed.(hope all these things are being done already).
