What Does This MRI Report Indicate?

I lost all use of my EHL tendon after a bunionectomy. The MRI report said: Postsurgical changes in the right foot with evidence of an osteotomy at the proximal shaft of the first metatarsal. Complete tear of the extensor hallucis longus tendon at the level of the osteotomy site, with a gap of approximately 21 mm. Translation, please. It seems likely that this occurred during surgery, yes?

General Surgeon 's  Response
Hello,

As per my clinical experience, the gap of 21 mm with osteotomy at proximal shaft of first metatarsal and tear of extensor hallucis longus tendon is related to surgery only. Most of the Bunions are firmly adherent to underlying bones and ligaments of surrounding architecture, the removal of which are necessary and essential during the surgical procedure.

This is a pre-operative assumption of the treating ortho surgeon and the same must have been discussed and explained thoroughly with the patient and first degree relatives. If the same has not been explained, medical negligence can be an issue of significant level in this regard and should be prevented to hospital ethics committee.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
