Hello and thank you for your question. Heterogeneous means that there are light and dark areas that show up on the MRI. Usually the appearance of bone is uniform, not spotty like this. When bone shows up spotty on MRI like this it often means that there is some kind of cancer going on. The only way to know for sure is to get a bone marrowbiopsy.
