What Does This MRI Showing A Disc Bulge Mean?

one inpression on my mri shows level of l4-5demonstrates mildbroad base disc bulge-borderline protusion with an annular tear to about the thecal sac. this closely approximates the s1 nerves b1 nerve roots bilaterally. and that there is mild to moderate narrowing of the medial border of the foramina. Can you please explain this to mean as in what it means, what can be done to fix whatever is wrong
