I have gone through your query.Your problem is most likely a rotator cuff tear of shoulder.It can be primarily managed non-operatively by physiotherapy analgesics and may be steroid injections.If physical therapy not helping to improve pain and function then you can opt for surgery.There are three types of surgery options- open surgery,mini open surgery and arthroscopic surgery.Hope this may help you.
