The presence of free fluid in pouch of douglas indicates possible bacterial infection most likely. The pelvic inflammatory condition may be provoked by various bugs but the most common ones are chlamydia trachomatis as well as neisseriagonorrhea. A cervical swab test is must for the final microbiology evaluation of the results and guidelines accordingly.
