Your semen analysis findings may indicate semen infection with low sperm count. Infection is indicated by presence of few leukocytes in semen. You may need investigations like routine hemogram, random blood sugar, semen culture and sensitivity. You may need to take antibiotics based on culture and sensitivity report.
Sperm count is low.It should be at least 20 millions/ml.Your findings suggests 12 millions/ml.
Motility should be at least 55 %.Your findings suggests over all 80% overall motility,so it is normal.
Sperm morphology is also normal.
Fertility is expected to increase with control of infection. If findings persists then you may need additional fertilization techniques.
