What Does Triple Marker Test And NT Combined 1:666 Suggest?

I am 38 and carrying my third baby.I am about to complete my 19th week. My elder son is 13 and younger one is 8. I had a normal NT scan and had triple marker test with 1:153 risk ratio for trisomy 21,which when combined with NT comes to 1:666. Is it a significant result? I'm otherwise normal and healthy.We've no history of any congenital abnormality in the family. Is TIFFA scan a must?
