Hello dear, thanks for your question on HCM.
I can understand your situation and problem.
In my opinion you should not worry at all for bilateral apical pleural thickening.
Because pleural thickening is suggestive of healed, scar tissue and not indicative of active lesion.
But better to get done CT thorax. Because CT thorax gives more superior view of thickening then chest x ray. It is also helpful in ruling out lung parenchymal involvement.
So if CT thorax is also suggestive of thickening then no need to worry at all.
And no need for anti TB drugs as it is not active TB lesion.
So be relax and calm.
