What Is The Diagnosis For The Painful Bump On Tongue And Polyp On Inner Cheek With Sore Throat?

I have noticed a painful growth or bump(looks kind of like a fat skin tag) on the left, back part of my tongue. almost to throat. I thought I was just getting a sore throat but when I looked to see if I had white on my throat, I realized that the pain was coming from my tongue. I stuck it out as far as possible and noticed the red bump. I also have a polyp, pinkie tip size, inside my right cheek. it has been there for a couple of years and is starting to become painful if touched. hope you can tell me what I should do and how soon. thank you.
