What Is The Reason For Swollen Head, Pressure And Pain After Head Surgery?

My husband had a metal plate put on his lt side of his head 7 yrs ago due to an accident he had tramatic brain injury he had a hematoma so had surgery and now has severe pain that lasts for 5 or 8 minutes then he feels a pressure sensation an feels like the lft side of his head is swollen why is that
