I was bitten by a big dog in May. I have two healed puncture wounds in my calf. I took a z pak right after the bite then I took a round of ciprofloxacin and 24 days of clindamycin in June/July. I still have hard lumps under the skin. One feels a bit tender. What could this be and do I need to go back to doctor?

It can be simple granulation tissue or scarring at the area which might be causing pain. It is quite commonly observed and there is nothing to be worried about the same. If it is not getting red or not oozing any fluid then there is nothing to be worried about.
The only precaution you must follow is not to touch, pinch pop or squeeze any of them other wise it can get infected and can cause complications.
If I were your treating dermatologist then would have suggested you take pain killer tablet by mouth for reducing the intensity of pain and redness. You can additionally take antihistamine benadryl or loratadine by mouth.
