What Is The Relation Between Ulcerative Colitis And Sclerosing Cholangitis?

I have been diagnosed with proctosigmoidities ulcerative colitis and i have heard that there is a strong relation between it and sclerosing cholangitis . My question is if i decide to do j-pouch operation which i think will end this disease , Does it also reduce the chance of getting sclerosing cholangitis or nothing is changed and Is it true that by doing j-pouch operation then there will be no longer auto-immune disease as the diseased organ has been removed and therefore no other anti-bodies will exist to destroy intestine or even liver ? thanks alot Dr.

Ulcerative colitis (UC) is an idiopathic inflammatory condition in the colon. About 30 percent of patients have the disease confined only in the recto-sigmoid region. Unfortunately, the cause of ulcerative colitis is unknown. UC has an association with sclerosing cholangitis.

It was observed that about 70 to 80% of patients with sclerosing cholangitis also have UC. On the other hand, only 4 to 5% of patients with UC have a risk to develop sclerosing cholangitis.
It’s hypothesized that the immune system may be one the causes of ulcerative colitis and for sclerosing cholangitis. This theory is based on the treatment of UC with immunosuppressant medications which showed some degree of effectiveness to control the disease.

Surgical management of chronic ulcerative colitis is achieved with Ileal pouch-anal anastomosis (IPAA) operation. J-pouch surgery is a type of IPAA surgery. Unfortunately, research shows that patients who undergo the IPAA surgery have a higher risk of developing sclerosing cholangitis.
