Ulcerative colitis (UC) is an idiopathic inflammatory condition in the
colon. About 30 percent of patients have the disease confined only in the recto-sigmoid region. Unfortunately, the cause of ulcerative colitis is unknown. UC has an association with
sclerosing cholangitis.
It was observed that about 70 to 80% of patients with sclerosing cholangitis also have UC. On the other hand, only 4 to 5% of patients with UC have a risk to develop sclerosing cholangitis.
It’s hypothesized that the immune system may be one the causes of ulcerative colitis and for sclerosing cholangitis. This theory is based on the treatment of UC with immunosuppressant medications which showed some degree of effectiveness to control the disease.
Surgical management of chronic ulcerative colitis is achieved with Ileal pouch-anal
anastomosis (IPAA) operation. J-pouch surgery is a type of IPAA surgery. Unfortunately, research shows that patients who undergo the IPAA surgery have a higher risk of developing sclerosing cholangitis.
