Hello & Welcome to HCM,

I had gone through the case and found that as he has congenital heart failure, so there are many cause of swollen of foot.

1- Water retention, for this give him diuretic.

2- Deep vein thrombosis, which causes decreased circulation of blood to leg due to that there is lot of pain with swelling.

For conformation go for Doppler study of vein and artery of leg. After that take medication for thrombosis.

Hope my answer will be effective for you.

Thanks,
