What Is The Remedy For Severe Itchy Rash On Entire Back After Taking Warfarin For Atrial Fibrilation?

My mother was recently diagnosed with Atril Fibrilation and was prescribed Warfarin. She has developed a severe and persistant rash on her back from the nape of her neck to her buttocks and onto her hips. It itches like mad and she s scratching her skin off. I m giving her benedryl orally and using hydrocordisone cream which helps some, but the rash/itching persists. How serious is this and can it be reversed?
