I think you need to focus on other activities besides
masturbation and especially at times when you feel like masturbating, think of some other things to do and move away from the act. Do not try to stop all at once, just slow it down at first and then stop. Set yourself a time period. And make that a realistic one.
Prepare a list of chores to do, and whenever you get the urge, concentrate on that. There are chances that the
backache is also due to the habit of masturbation - yes, the incidence is known. So, I feel once you reduce the masturbation, the backache may reduce.
As for gym, I think make a start. Don't push yourself, go slow. Once you start, the muscles will gradually strengthen and then the exercise tolerance will increase. You may be able to step up the level of exercise thereafter.
As for sperm quality, get a sperm test/analysis done. Then we can give a better comment on the latter part of your query.
