Hi there I am Dr Mittal I have read your query I understand the gravity of your problem I will try to help you as far as possible The fact remains that sugars need to be well controlled The main aspect of wet gangrene control needs sugars to be well controlled No antibiotic would help till sugars are controlled The best way to control sugars would be using insulin. I prefer, in my patients, to give half the needed insulin in the infected site of wet gangrene. A culture from wet gangrene amputation site should be done to guide the antibiotic cover.
