What Is The Treatment For Discharge From Nipples?

I'm 13 weeks pregnant and I have inflamed red sensitive skin around the outside of my areolas on both breasts. I have been wearing underwire bras. Also I have a discharge on both nipples... I'm in pain and I'm not sure if this is normal. I have always had pimple like substances on my areola that when I squeezed them they had white stuff come out but at the moment it is on the actual nipple and yellow to brown colour
