Hi,
Noted your history of ear drum peroration, painful syringing, drill out operation by an ENT for swimmer's ear and so on.
Peroration can be due to trauma and most usually is due to middle
ear infection causing pressure and perforation.
Additionally you have swimmer's ear which needs a long treatment, care not to allow water in ear, anti-fungal drops, anti-inflammatory, decongestants if needed and any other treatment as wouldbe advised.
You will have to get the blood tests to rule out
diabetes.
Proper care and management will normalize the ear.
Once all these thing are settled you can go for definitive surgery.
Get
CT scan of mastoids done to be sure about the internal changes.
