My son 3.5 years old is suffering with middle ear fluid issue and is going to be operated for removal of adenoids. The confusion here is that one surgeon has suggested that he has to undergo myringotomy as well while the other says that the middle ear fluid issue can be treated without operation by medicine and in most cases it (middle ear fluid) is gone just like that.

As per your complain it seems that accumulation of fluid in middle ear occurs most commonly due to infections like cold, ear infections etc.. It is true that the fluid in middle ear can be treated with medications and he can be advised to take a course of antibiotics, anti-inflammatory painkillers and decongestants oral as well as nasal spray..
Surgical drainage or Myringotomy is done in which a small incision/cut is made in the eardrum followed by drainage of fluids and in severe cases pressure equlization tubes are also placed.. It is done only if after prolonged treatment the fluid does not get drained off leading to continuous pain and clogged feeling and even the hearing can be effected..
So my suggestion would be to get a conservative treatment that is by medications done and if there is no relief then only proceed with surgical drainage..
