I understand your concern
Very painful menstrual cramp could be due to primary/spasmodic type, secondary causes like pelvic inflammatory disorder,
vaginal infection,
endometriosis.
I would suggest to consult gynecologist and undergo examination, reproductive hormone analysis, USG scan, blood sugar , urine tests, CBC to rule out cause.
Specific treatment depend on cause
Birth control pill will help you to decrease pain
Drink plenty of water/juice
Take healthy diet with lots of fresh fruits and green leafy vegetables in small frequent meal
Avoid: high salt/fatty/oily food, alcohol,
smoking, tea, coffee in second half of period
Take physiotherapist advise to increase pain threshold
Do regular exercise like walking, running followed by medication to combat
stress, maintain proper pelvic hygiene
If pain is not tolerablet then you can take tablet meftals spas for 2-3 days
