Hello,
Thanks for choosing health care magic for posting your query.
I have gone through your question in detail and I can understand what you are going through.
If you have a history of pots spine and have recent pain in D8 region then you may get a fresh MRI of the whole spine done to rule out if there is any active lesion in D8 area. Regarding tiredness on exertion, its possible that you may be having a depressive episode which makes you smoke more often and also reduce your exercise capability. Its would be wise to get consulted by a psychiatrist ones.
Hope I am able to answer your concerns.
If you have any further query, I would be glad to help you.
