What Should Be The Correct Dose Of Lactogen For 3 Months Old ?

Hi I have a baby girl who is 12 weeks 5 days (3 month )old. Her birth weight was 3.3kg but within a week she was 2.9kg. I was completely breast feeding her till 2 weeks back but her weight was only 3.7kg(2.5 month old) So the Doctor suggested to give formula milk along with my milk. I have started giving lactogen for 2 weeks(60ml - 3 times a day) and now she is 4.2 kgs. I am not sure how many times i have to give formula milk and the quantity along with breast feeding. I am concerned about her weight because she is under weight for 3 months.
